chore(index): remove contextInjector chat.message hook call

- Remove createContextInjectorHook from imports
- Remove contextInjector variable declaration
- Remove contextInjector["chat.message"] call
- Keep contextInjectorMessagesTransform for synthetic part injection
- Update test: prepend → synthetic part insertion verification
